   By completing and submitting this form at the Dental On Demand PTY LTD booth,  you consent to being contacted for marketing and promotional purposes by Dental On Demand PTY LTD. You may receive information about our services, offers, and updates via email, phone, or SMS.

2. About Our Services
   Dental On Demand PTY LTD provides mobile dental services, bringing dental care directly to you. We service childcare centres, aged care facilities, home visits, schools, independent living communities, and corporate settings. Our portable dental units can be set up in most locations, making treatment convenient and accessible.

3. Privacy and Data Use
   Any personal information collected through this form will be handled in accordance with Dental On Demand PTY LTD’s Privacy Policy and applicable Australian privacy laws. Your information will not be shared or sold to third parties without your consent, except where required by law.

4. Prize Draw
   From time to time, Dental On Demand PTY LTD may conduct a prize draw for attendees who submit this form. Entry is limited to one submission per person, and each entrant may only win once per prize draw. Winners will be selected at random from eligible entries and notified using the contact details provided. Prizes will be delivered as advised at the booth. No substitutions will be offered for prizes unless stated otherwise.

5. Eligibility
   Entrants must be over 18 years of age and attend the Exhibition Booth or event in person. Employees, contractors, and representatives of Dental On Demand PTY LTD, along with their immediate family members, are not eligible to participate in prize draws.

6. Prize Collection
   Winners must respond within the timeframe stated in the notification to claim their prize. Dental On Demand PTY LTD is not responsible for unclaimed prizes.

7. Liability
   Dental On Demand PTY LTD will not be held liable for any loss, damage, or injury arising from participation in a prize draw or use of a prize. We accept no responsibility for incomplete entries, form submission errors, technical issues, or other factors that may prevent entry or successful contact.

8. Right to Modify or Cancel
   Dental On Demand PTY LTD reserves the right to amend, suspend, or cancel these terms and conditions, or any aspect of a prize draw, at any time without prior notice.

9. Governing Law
   These terms and conditions are governed by the laws of New South Wales, Australia.
