Dental On Demand PTY LTD – Sydney Care Expo Terms and Conditions

1. Purpose of Consent Form
   By completing and submitting the form at the Dental On Demand PTY LTD booth, you consent to being contacted for marketing and promotional purposes by Dental On Demand PTY LTD. You may receive information about services, offers, and updates through various contact methods, including email, phone, or SMS.

2. Privacy and Data Use
   Any personal information collected through this form will be handled in accordance with Dental On Demand PTY LTD's Privacy Policy and applicable Australian privacy laws. We will not share or sell your information to third parties without your consent, except as required by law.

3. Prize Draw
   A prize draw will be held on each day of the Expo on 15th and 16th November 2024. Each prize will be drawn at 6:00 PM daily from that day’s entries. Attendees are advised to enter only once. Each entrant may only win once across both days. The winner will be notified via email and phone using the contact details provided on the form. The prize will be posted to the winner shortly after the Expo. No substitutions will be offered for the prize as described at the booth.

4. Eligibility
   Entrants must be over 18 years of age and attend the Sydney Care Expo in person. Employees and representatives of Dental On Demand PTY LTD and their immediate family members are not eligible to win the prize.

5. Prize Collection
   Winners must respond to notification within 30 days of the Expo's conclusion to claim their prize. Dental On Demand PTY LTD is not responsible for any unclaimed prizes.

6. Liability
   Dental On Demand PTY LTD will not be held liable for any loss, damage, or injury related to the prize or its use. Dental On Demand PTY LTD also accepts no responsibility for any form submission errors, technical failures, or other issues that may prevent entry into the draw or contact for marketing purposes.

7. Right to Modify or Cancel
   Dental On Demand PTY LTD reserves the right to modify or cancel these terms and conditions or any element of the prize draw at any time without notice.

8. Governing Law
   These terms and conditions are governed by the laws of New South Wales, Australia
